Further Description:
We are currently looking for a dedicated and committed Social Worker (Adults). This position is to work within the Portsmouth Discharge to Assess Team, completing Care Act assessment with clients who have been discharged from Hospital. The Team covers 2 discharge to assess units, residential and nursing homes and with people in their own homes. This is a fast paced role with the work being short term. Working knowledge of the CHC process is also required as there is an expectation to complete checklists and attend DST meetings where required.

- Registration in Social Work England
