Further Description:
We are currently looking for a committed and dedicated Asw Social Worker to join our Older Adult Community Mental health team. The role is working with older adults with physical and mental health problems. Assessments and Reviews, Discharge planning, Crisis intervention. Also working with service users who have a dementia or a functional mental health problem, depression or schizophrenia. Multidisciplinary working. Working with strength-based model with services users and carers. Funding panel and placement work.

- Registration with Social Work England
