Further Description:
We are looking for passionate and experienced Adult Social Workers to undertake care and support assessments, mental capacity assessments and reviews. This is a mixture of visits face to face and remote dependent on the requirements in each case. To make referrals to partner agencies To liaise with internal services. Potentially to undertake Community Dol's if required. To undertake reviews following hospital discharge. To support care coordinators in the team as required. To undertake welfare visits.
Essential Criteria:
- Social Work Qualification
- Registration in Social Work England
