Further Description:
We are currently in need of a Qualified Social Worker for First Response Adults to join our team based in London Borough Of Merton
The post is within the Adults First Response Team based here at Civic Centre. I would prefer someone who is a driver and has access to a car as they will be expected to undertake some visits across the borough. PPE is provided. They need to have experience of Care Act assessments, NHS Continuing Healthcare assessments, Safeguarding Adults Enquiries in a community setting and be able to undertake assessment and care management functions including presenting to Outcomes Forum for funding decisions.
